Hamstring Strains: Why Sport and Dance Require Different Rehab

The hamstrings are a powerful muscle group on the back of the thigh comprising four distinct muscle bellies. They play a critical role in walking, running, jumping, and high-level athletic performance.

However, hamstring injuries remain one of the most persistent issues in active populations, accounting for approximately 10% of all injuries in field-based team sports according to recent clinical research (Maniar et al., 2023).

While muscle tears (strains) are common across both sports and performing arts, the underlying mechanical cause varies significantly depending on the activity. Understanding these distinctions allows us to design precise, evidence-based rehabilitation protocols at Activate.

1. The Mechanism in Sport: High-Speed Deceleration

In running and field sports, hamstring strains rarely occur when the leg is simply pushing off the ground. Instead, they predominantly occur during the “late-swing” phase of sprinting—the moment right before the front foot strikes the turf.

In this phase, two things happen at once:

  • The hamstring is stretched long as the hip flexes.
  • The muscle must contract rapidly and forcefully to decelerate the forward movement of the lower leg.

Think of a stretched rubber band. When a muscle is fully lengthened and forced to absorb high mechanical energy, it becomes highly vulnerable to tearing if its load capacity is exceeded.

2. The Mechanism in Dance: Extreme End-Range Overstretch

In contrast to field sports, dancers infrequently suffer from high-speed deceleration injuries. Instead, dancers experience hamstring strains caused by positional overstretching.

Dance disciplines regularly require performers to move their limbs into extreme ranges of motion—such as high kicks (grand battements), splits, or Arabesques. When the hamstring is pulled to its absolute limit, the muscle fibers can experience micro-tears, particularly near the ischial tuberosity (sit bone).

1. Personalised Heavy Load & Kinetic Chain Strengthening

We progressively build strength in the hamstrings through specific eccentric exercises (strengthening while lengthening). Additionally, we address the surrounding kinetic chain—including the gluteals and deep core muscles—to ensure the hamstrings aren’t overworking due to pelvic instability.

2. Graded Range of Motion

Stretching an acutely injured muscle can aggravate the tear. We utilize a carefully monitored, graded reintroduction of mobility exercise to ensure the muscle tissue heals with optimal elasticity without triggering protective muscle spasms.

3. Coordinated Return-to-Activity Plans

Rehabilitation doesn’t happen in isolation. Our clinical team actively collaborates with your sports coaches, dance teachers, and parents to modify training loads. This ensures you stay active and engaged in your sport or studio while protecting the healing tissue.
